Below are the important documents members and prospective members should read.
- Product Disclosure Statement: Contains all the important information needed to make an informed decision about joining Cruelty Free Super.
- Financial Services Guide: Contains important information about who we are and your rights as a client.
- Additional Information Booklet: Contains important information about Cruelty Free Super, as well as additional information about superannuation and tax.
- Insurance Guide: Contains important information about the insurance offerings available to Cruelty Free Super members.
- Significant Event Notice: If there's been a recent significant change, this document will contain important information about an upcoming or recent change to the structure of the fund.
- Grosvenor Pirie Master Trust No. 2 Trust Deed: Sets out the rules we follow as a super fund, and what the trustee can and can’t do.
Forms for individuals
Below are the forms you will need to become a member of Cruelty Free Super, as well as forms regarding rollovers, withdrawals, insurance and other transactions with the fund.
Note: the quickest and easiest way to become a member of Cruelty Free Super, and rollover from your old super fund at the same time, is to fill out the online Application Form.
- Application Form: Complete this form to become a member of the Cruelty Free Super fund. Alternatively, you can download the printable Application Form.
- General Compliance Certificate: Employers will require this certificate to set up contributions into your Cruelty Free Super fund.
- Rollover Form: Complete this form if you are already a member of the Cruelty Free Super fund, and would like to rollover some money from another super fund into your Cruelty Free Super account.
- Standard Choice Form: Employers may require this form to be completed when an employee chooses to join Cruelty Free Super.
- Personal Contribution Form: Complete this form if you would like to make a personal contribution of money into your Cruelty Free Super account. If you are making a self-employed concessional contribution (before-tax), you must also complete the Notice of intent to claim or vary a deduction for personal super contributions form and send it to us with your Personal Contribution Form.
- Life Insurance Application Form: Complete this form if you would like to access death, total permanent disablement, or income protection insurance through your Cruelty Free Super account. Please contact us on 1300 022 762 or at hello@crueltyfreesuperannuation.com.au for this form.
- Life Insurance Transfer Form: Complete this form if you would like to transfer your current insurance coverage from another fund to your Cruelty Free Super account. Please contact us on 1300 022 762 or at hello@crueltyfreesuperannuation.com.au for this form.
- Non-Binding Nomination of Beneficiaries Form: Complete this form to inform the trustee about who you would like to receive the money in your Cruelty Free Super account in the case of your death.
- Binding Nomination of Beneficiaries Form: Complete this form to inform the trustee about who you would like to receive the money in your Cruelty Free Super account in the case of your death.
- Withdrawal Form: Complete this form if you would like to apply to withdraw all or some of your super money.
- Early Release Due To Financial Hardship Form: Complete this form if you would like to access all or some of your super money due to financial hardship. To access funds for severe financial hardship, you will also need to complete a Cruelty Free Super Withdrawal Form.
- Early Release Due to Permanent Incapacity: Complete this form if you would like to access all or some of your super money as you are unable to work due to a medical condition.
- Adviser Services Form: Complete this form if you have a financial adviser and you would like them to have access to your account information. You can also complete this form if you would like an Adviser Service Fee paid from your super account to your financial adviser.
- Change of Details Form: Complete this form to update your personal details with Cruelty Free Super.
- Contribution Splitting Form: Complete this form to split your superannuation contributions.
- Work test declaration: Complete this form if you are over the age of 65 and would like to make a personal contribution into your Cruelty Free Super account. Please contact Cruelty Free Super with further questions regarding the work test and how it applies to you. - Adviser Dealer Group registration: Complete this form to register your dealer group with Cruelty Free Super in order to be paid an adviser services fee through your client’s account.
